Resumo(s)Purpose: The purpose of this study was to assess the changes in curvature along the horizontal meridian of the anterior cornea occurring after refractive surgery and corneal refractive therapy (CRT). Methods: One hundred twenty-two eyes of 122 patients (70 female) with a mean (SD) age of 30.6±7.5 years were retrospectively analyzed in this study. Forty-three of those underwent standard LASIK ablation, 40 had customized LASIK and 39 had corneal refractive therapy (CRT) with orthokeratology lenses. Patients in different groups were matched in order to be comparable in terms of pretreatment refractive error and corneal topographic profiles. Topographical data along the horizontal meridian were collected over a 10mm corneal diameter in 1mm steps using the tangential power map from the computer display. Results: A statistically significant increase in corneal power (p<0.05) was observed at the nasal and temporal locations between 2 and 3 mm for CRT and 3 and 4mm for LASIK procedures, from corneal keratometric center. This steepening was more pronounced after CRT treatment (p<0.05 for all comparisons against LASIK groups), but not significantly different between LASIK groups (p>0.124, Mann-Whitney Test). Conclusions: Both, surgical and non-surgical interventions show a mid-peripheral local corneal steepening. However, the narrower optic zone and higher midperipheral steepening with CRT seems to provide the potential to create a more relative peripheral myopic increase in corneal power than LASIK, which may have implications in slowing down myopia progression.
Objectivo: O objectivo deste estudo foi avaliar as alterações que ocorrem na curvatura da superfície anterior da córnea ao longo do meridiano horizontal após a cirurgia refractiva LASIK e a terapia refractiva corneal (TRC). Métodos: Cento e vinte e dois olhos de 122 pacientes (70 mulheres) com idade média ± DP de 30,6 ± 7,5 anos foram analisados retrospectivamente neste estudo. Quarenta e três deles foram submetidos a uma ablação LASIK standard (LS), 40 foram submetidos a LASIK personalizado (LP) e 39 a TRC com lentes de ortoqueratologia (ortoK). Os pacientes para os diferentes grupos foram seleccionados para serem comparáveis em termos de erro refractivo e nos perfis topográficos da superfície anterior da córnea (SAC) antes dos respectivos tratamentos. Os dados topográficos ao longo do meridiano horizontal foram analisados num diâmetro de 10 milímetros de córnea em passos de um milímetro, usando o mapa de potência tangencial. Resultados: Foi observado um aumento estatisticamente significativo na potência da córnea (p < 0,05) nos pontos nasal e temporal entre 2 e 3 mm para TRC e 3 e 4 mm para os procedimentos de LASIK, a partir do centro queratométrico da córnea. Esta tendência foi mais acentuada após o tratamento TRC (p < 0,05 em comparação com os grupos LASIK), mas não significativa- mente diferente entre os grupos LASIK (p > 0,124, Mann-Whitney). Conclusões: Ambas as intervenções cirúrgicas e não cirúrgicas mostram um incremento de potência localizado na zona média da SAC. No entanto, a zona óptica mais pequena e uma maior curvatura na periferia média com a TRC parece fornecer o potencial para criar um efeito miópico periférico relativo na potência da córnea maior do que no tratamento LASIK, e assim ter um maior potencial de atraso na progressão da miopia.
