Resumo(s): | De todo o sistema musculosquelético, a articulação tibiotársica é a que regista maior incidência de
lesões de todas as articulações do corpo humano quer durante a realização de atividades diárias como na
prática desportiva. A constante solicitação funcional do tornozelo, especialmente na realização de exercício
físico ao qual está associada constantemente a necessidade de elevação do corpo face ao solo, tem como
principal consequência a exposição dessa estrutura a condições críticas adversas aos requisitos ideais de
equilíbrio e estabilização.
Um dos obstáculos ainda muito comum atualmente é o facto das etapas de análise e diagnóstico
dessas lesões serem realizadas durante duas fases distintas: um exame manual feito pelo médico ortopedista
com o intuito de avaliar a capacidade funcional recorrendo à simulação do movimento e à palpação da zona
periférica da dor, e por um exame imagiológico, onde é avaliada a condição anatómica desse membro através
da análise da posição relativa dos elementos.
O facto dos dois exames serem realizados em espaços de tempo diferentes dificulta a convergência
dos resultados obtidos, diminuindo significativamente o efeito de complementaridade ao qual os dois deveriam
estar associados. Por outro lado, com o recurso a este tipo de técnicas de diagnóstico não é possível alcançar
o rigor e a reprodutibilidade que são indispensáveis num processo desta natureza. Assim, com o objetivo de
facilitar e otimizar o processo de diagnóstico, tanto do ponto de vista do doente, como do dos radiólogos e
ortopedistas, apresenta-se neste trabalho o projeto de um dispositivo médico para apoio ao diagnóstico de
patologias no tornozelo.
Para o desenvolvimento do projeto foram estabelecidos os objetivos do trabalho de forma a definir as
caraterísticas do produto final. Nesse sentido foram realizadas várias reuniões com a equipa de trabalho onde
se estabeleceram os requisitos a que o produto deveria obedecer e que darão origem às especificações
técnicas.
Para o projeto concetual o dispositivo foi dividido em três subsistemas para os quais foram propostas
diferentes soluções, que mediante a sua viabilidade foram, ou não, modeladas tridimensionalmente. Depois de
escolhida a melhor solução para cada subsistema ficou definida a solução final.
Com a solução adotada é possível associar a um exame imagiológico de precisão, como a Ressonância
Magnética, um esforço físico. Desta forma permite a quantificação do limite móbil da estrutura de um tornozelo,
o que constitui uma mais-valia para os profissionais de saúde e para os pacientes. Of the entire musculoskeletal system, the hock joint is the one that registers the highest incidence of injuries of all joints in the human body, not only during daily activities but also in sports. The constant functional request of the ankle, especially during the practice of sports activities which are constantly associated with the need to raise the body relatively to the ground, has as main consequence the exposure of this structure to critical conditions that are the opposite of the ideal requirements of balance and stabilization. One of the obstacles that is still very common nowadays is the fact that the analysis and diagnosis stages of these injuries are performed during two distinct phases: a manual examination done by an orthopedist in order to evaluate the functional capacity using the simulation of movement and palpation of the peripheral pain area, and through an imaging examination, where the anatomical condition of that member is evaluated by the analysis of the relative position of the elements. The fact that the two tests are performed in different time slots, makes the convergence of the results obtained difficult, reducing significantly the effect of complementarity to which both should be associated. Moreover, with the use of such diagnostic techniques the accuracy and reproducibility that are essential in a process of this nature can’t be achieved. Thus, in order to ease and optimize the diagnostic process, the point of view of the patient's and also of radiologists and orthopedists is considered, and this way in this paper the design of a medical device to support the diagnosis of ankle pathologies is presented. For the development of the project the goals of the work were established with the aim of define the final characteristics of the device. To achieve and define the main goals of the project several meetings were held with the work group, establishing the requirements that the product should obey and that will form the technical specifications. For the conceptual design, the device has been divided in three subsystems for which different solutions have been proposed, and depending of their viability, were or not three-dimensionally modeled. After the selection of the best solution for each subsystem, the final solution for the device became fully defined. With the adopted solution is possible to associate a physical effort to a precision imaging examination, such as MRI. This way it allows the quantification of the mobile limit of the ankle structure, which is an asset for health professionals and for the patients. |
