Resumo(s)Os sintomas de secura ocular afetam milhões de pessoas em todo o mundo e as lágrimas artificiais são hoje em dia extensamente utilizadas para o tratamento destes sintomas, hidratação, lubrificação e limpeza dos olhos. O objetivo deste trabalho foi avaliar as alterações dinâmicas na estabilidade do filme lacrimal baseada em parâmetros obtidos através da realização da topografia corneal depois de instilar lágrima artificial num grupo de sujeitos. Pretendeu-se avaliar as diferenças nos respetivos parâmetros após instilação de diferentes lágrimas disponíveis comercialmente para caraterizar a duração da sua atividade e as diferenças entre elas. Foram selecionados 32 pacientes, aos quais foram avaliados parâmetros da pelicula lacrimal através da realização da topografia corneal. As medidas foram realizadas inicialmente (sem lágrima) e após a colocação de lágrima, ao fim de 5, 15 e 30 minutos. As lágrimas em estudo foram a Systane Ultra (Alcon, Novartis Company), Systane Ultra Plus (Alcon, Novartis Company) e HyalDrop (Bausch & Lomb Company), que apresentam atualmente grande implementação no mercado português. Para a tomada de medidas foi utilizado o topógrafo Medmont E-300 (Medmont Pty.,Ltd., Melbourne, Austrália), com recurso a um software de análise da película lacrimal que fornece índices de avaliação da qualidade da superfície lacrimal, Tear Film Surface Quality (TFSQ), tempo de rutura lacrimal não invasivo (TRLNI) e área da rutura lacrimal. O inquérito Ocular Surface Disease Index (OSDI) foi aplicado inicialmente para posterior comparação de resultados dos sintomas apresentados. Os resultados mostram que o TRLNI medido com recurso ao topógrafo de forma automática são mais baixos do que os medidos de forma manual (p <0.001). Após a instilação de lágrima artificial o índice TFSQ sofre alterações na forma imediata, mostrando aumento de qualidade na pelicula lacrimal (p <0.001) apesar dos valores de TRLNI se manterem sem alterações significativas (p> 0.05). As lágrimas Systane Ultra e Systane Ultra Plus são as que apresentam melhores resultados imediatamente após a colocação, no entanto ao fim de 5 minutos de aplicação todas as lágrimas perdem a sua eficácia em termos de estabilidade lacrimal. Não foi encontrada correlação entre o valor do OSDI com o TRLNI ou com o TFSQ. Tratando-se o TFSQ de um índice de avaliação da superfície lacrimal novo e mostrando os valores de TRLNI serem tão baixos quando comparativamente com os medidos de forma manual, novos estudos devem ser feitos por forma a clarificar de que forma nos podem ajudar na deteção e tratamento do olho seco de uma forma mais eficaz.
Symptoms of dry eye affect millions of people around the world and artificial tears are now widely used for the treatment of these symptoms, moisturizing, lubricating and cleaning the eyes. The objective of this work was to evaluate the dynamic changes in the stability of the lacrimal film based on parameters obtained through the corneal topography after instilling artificial tears in a group of subjects. It was intended to evaluate the differences in the respective parameters after instillation of different commercially available tears to characterize the duration of their activity and the differences between them. Thirty-two patients were selected, and parameters of the tear film were evaluated through corneal topography. The measurements were performed initially (without tear) and after tear instillation, after 5, 15 and 30 minutes. The tears in study were Systane Ultra (Alcon, Novartis Company), Systane Ultra Plus (Alcon, Novartis Company) and HyalDrop (Bausch & Lomb Company), which currently have great implementation in the Portuguese market. The Medmont E-300 corneal topographer (Medmont Pty.,Ltd, Melbourne, Australia) was used for the measurement using a tear film analysis software that provides a lacrimal surface quality evaluation indices, Tear Film Surface Quality (TFSQ), non-invasive break-up time (NIBUT) and lacrimal rupture area. The Ocular Surface Disease Index (OSDI) survey was initially applied for later comparison of the results of the presented symptoms. The results show that the TRLNI measured automatically using the corneal topographer are lower than those measured manually (p <0.001). After tear instillation, the TFSQ index changes immediately, showing an increase in lacrimal film quality (p <0.001), although the NIBUT values remained unchanged (p >0.05). The tears Systane Ultra and Systane Ultra Plus are the ones that present better results immediately after the instillation, however after 5 minutes of application all the tears lose their effectiveness. No correlation was found between the values of OSDI with NIBUT or TFSQ, Since the TFSQ is a new lacrimal surface index and the NIBUT values are so low compared to those measured manually, further studies should be done to clarify how they can help us detect and treatment of dry eye in a more effective way.
