Resumo(s): | A presente tese é composta por três estudos desenvolvidos no âmbito da investigação focada
na compreensão do processo terapêutico, especificamente em torno da relação entre processos
interativos intra sessão e a qualidade da aliança terapêutica. Nestes três estudos, abordamos como
referências teóricas, o conceito de Colaboração terapêutica desenvolvido por Ribeiro e colaboradores
(2013) baseado no conceito de Zona de Desenvolvimento Proximal terapêutica (ZDPT) apropriado à
psicoterapia por Leiman & Stiles (2001), e o conceito de aliança terapêutica transteórico, proposto por
Bordin (1979).
A Zona de Desenvolvimento Proximal Terapêutica (ZDPT) representa o espaço hipotético
compreendido entre o atual nível de desenvolvimento cognitivo e emocional do cliente até o nível
seguinte, idealmente compreendido como potencial e que pode ser alcançado com a ajuda do
terapeuta ao longo da terapia, através de uma interação equilibrada e colaborativa.
Recorremos ao Sistema de Codificação da Colaboração Terapêutica (SCCT; Ribeiro et. al,
2013) para avaliar e identificar como a colaboração é estabelecida e negociada entre os participantes
ao longo da conversação terapêutica. O SCCT (2013) permite identificar três diferentes intervenções
terapêuticas, assim como, cinco diferentes respostas do cliente para cada intervenção do terapeuta.
A combinação das intervenções e as respostas do cliente permitem identificar quinze diferentes
episódios interativos que se diferenciam em colaborativos, não-colaborativos ou ambivalentes,
considerando os limites da Zona de Desenvolvimento Proximal terapêutica do cliente.
O primeiro estudo desta tese, através de uma revisão sistemática de literatura, teve como
objetivo identificar e descrever os principais tipos de estudos empíricos, no período de 2000 a 2016,
que relacionassem a qualidade da aliança terapêutica com os processos colaborativos, identificados
intra sessão. Foram selecionados onze estudos nos quais a aliança terapêutica e os processos
interativos são analisados num design de estudo misto, tanto qualitativo quanto quantitativo. Os
resultados encontrados indicam que os estudos focados nos processos interativos e sua relação com desenvolvimento da aliança terapêutica são ainda escassos e privilegiam abordagens baseadas na
observação externa.
O segundo estudo teve como principal objetivo analisar como a colaboração terapêutica intra
sessão e a aliança terapêutica avaliada pelo cliente ao fim de cada sessão, variavam ao longo do
tempo em oito casos clínicos de resultado terapêutico contrastantes em Terapia Cognitivo-
Comportamental e quais os episódios colaborativos identificados pelo SCCT afetavam na avaliação da
aliança terapêutica. Em geral, o grupo de sucesso terapêutico trabalhou em direção e mais próximo
do nível de desenvolvimento potencial do cliente, através de intervenções de ‘desafio’, validadas pelos
clientes. Em contrapartida, o grupo de insucesso focou-se a maior parte do tempo do trabalho
terapêutico em torno da perspetiva problemática do cliente, em que a intervenção do terapeuta, sendo
de suporte, e por isso focada no nível de desenvolvimento atual do cliente, era validada pelo mesmo.
As análises longitudinais revelaram que o episódio de ‘desafio-segurança’ no grupo de sucesso
terapêutico, contribuía para o aumento da qualidade na avaliação da aliança, contudo os episódios de
‘suporte no problema-segurança’ e ‘desafio-risco intolerável’ contribuíram negativamente para a
avaliação da aliança terapêutica.
Por fim, no terceiro estudo, tivemos como objetivo identificar quais os episódios colaborativos
mais prováveis de promover o resultado terapêutico. As análises longitudinais revelaram que os
episódios de ‘desafio-segurança’ e ‘desafio-risco tolerável’ aumentavam a probabilidade de ocorrência
do sucesso terapêutico, todavia, os episódios de ‘desafio-ambivalência’ e ‘desafio-risco intolerável’,
expectados como preditores do insucesso terapêutico, não se revelaram, estatisticamente,
significativos. E ainda, o episódio de ‘suporte no problema-segurança’ revelou ser um preditor negativo
do insucesso terapêutico.
Em suma, os resultados encontrados ao longo desses três estudos, sugerem que nos casos
clínicos considerados de Sucesso terapêutico, os terapeutas trabalharam de forma mais responsiva e
atenta às capacidades emocionais e cognitivas dos clientes, promovendo mais avanço na Zona de
desenvolvimento do que nos casos clínicos de Insucesso terapêutico. Além disso, concluímos que as
interações em direção ao nível potencial do cliente são primordiais para a promoção da mudança
terapêutica e para o fortalecimento da aliança terapêutica. This Thesis is developed on three studies focused in the investigation and understanding of the therapeutic process, specifically towards the relation between the interactive processes in session and the quality of the therapeutic alliance. In these three studies, we are taking as theoretical references, the therapeutic Collaboration concept developed by Ribeiro and authors (2013) based on the Therapeutic Zone Developmental Proximal (TZDP) concept applied to the psychotherapy context by Leiman & Stiles (2001), as well as the therapeutic alliance concept proposed by E. Bordin (1979). The Therapeutic Zone Developmental Proximal represents the hypothetical space between the current level of cognitive and emotional development of the client until the next level. It’s ideally understood as potentially and highly functional, which can be reached with the therapist’s help throughout therapy, with a balance and collaborative interaction. We used the resources of the Therapeutic Collaboration Coding System (TCCS, Ribeiro et. al, 2013) to evaluate and to identify the establishment of the therapeutic Collaboration, as well as how the collaboration is negotiated between therapist and client throughout the therapeutic conversation. The TCCS allows us to identify three different therapeutic interventions, as well as, five different client’s responses for each therapist’s intervention. The combination between interventions and client’s responses indicate fifteen different therapeutic exchanges. These therapeutic exchanges can be identified as collaborative, non-collaborative or ambivalent, taking in account the limits of client’s TZDP. The first study was developed through a Systematic literature review. It aim to identify and to describe the main empirical studies types during the period of 2000 until 2016, that related the therapeutic alliance quality with the collaborative microprocesses occurred in session. Eleven empirical studies were selected, in which the therapeutic alliance and the interactive processes are analyzed in a mixed design, as well as qualitative study design or quantitative one. The results indicate that the studies focused in the interactive processes and in their relation with the therapeutic alliance’s development which are still scarce and concentrate in external observation approaches. The second study was to analyze how the therapeutic collaboration developed between the participants, and the therapeutic alliance evaluated by the client at the end of each session, variating throughout of time in eight complete clinical cases with contrasting therapeutic outcome in CBT, either with good or poor outcome. Besides that, we aimed identifying which therapeutic exchanges could affect in the evaluation of the therapeutic alliance. In general, the good therapeutic outcome group worked toward and nearest to the client’s potential level, emphasizing ‘challenges’ interventions, in turn validates by the client. The Poor therapeutic outcome group focused mostly on the therapy time around the client’s problematic perspective, emphasizing ‘support’ interventions approaching, in this way, in the current development of the client. The Longitudinal analysis showed that the therapeutic Exchange of ‘challenge-safety’ in the good outcome group, contributes to the increase of the evaluation of the alliance, however the therapeutic exchanges ‘support on problem-safety’ and ‘challengeintolerable risk’ promotes a negative effect in the therapeutic alliance. In addition, the third study, we aim identify which therapeutic exchanges are more probable to promotes the therapeutic outcome. The analysis showed that therapeutic exchange ‘challengesafety’ and ‘challenge-tolerable risk’ increased the probability of good therapeutic outcome, however the episodes of ‘challenge-ambivalence’ and ‘challenge-intolerable risk’ did not reveal statistically significant. Moreover, the therapeutic exchange ‘support on problem-safety’ revealed a negative predictor of poor therapeutic outcome. Finally, the results founded in these three studies, indicate that in clinical cases considered good outcome, the therapists worked more responsively and attentively regarding the emotional and cognitive capacities of the clients, promoting more advance in the Zone of development than in the poor outcome cases. Moreover, we concluded that interactions in direction to client’s potential level are fundamental to promoting therapeutic change and to strengthen of the therapeutic alliance. |
