Resumo(s)A interface entre gênero e saúde mental ainda não é explorada de forma satisfatória. Seja em termos de pesquisas ou de práticas interventivas, tais entrelaçamentos são insuficientemente abordados, o que causa implicações desfavoráveis especialmente às pessoas que vivenciam algum sofrimento psíquico. Ainda é parco o investimento sobre esta relação para além de questões biomédicas. Por isso, convém aprofundar o conhecimento dos modos como o gênero se faz presente nas experiências e nos significados que a doença mental tem para aqueles/as que dela padecem. Incluir a dimensão (inter)subjetiva dos sujeitos está de acordo com as práticas impulsionadas pela reforma psiquiátrica que têm remodelado o modelo de atenção à saúde mental no Brasil. Os princípios e valores reformistas têm investido nas temáticas sobre o sujeito, a subjetividade, as interações humanas, a experiência vivida com a doença, as alternativas terapêuticas. Assim, buscamos analisar, descrever e compreender: o que as pessoas que vivenciam algum sofrimento psíquico acham que têm/sentem/consideram ou não uma doença? Que sintomas enunciam? Que “causas” atribuem à sua condição? Que mudanças na vida percebem a partir da doença? O que fazem para lidar com a doença e o estigma? Que expectativas tecem sobre o futuro? Que concepções têm acerca de como outros/as homens e mulheres podem adoecer? Esta investigação teve caráter qualitativo e procurou analisar, descrever e compreender as experiências de homens e mulheres diagnosticado/as com doença mental, usuários/as de um Centro de Atenção Psicossocial II (CAPS II), serviço de saúde mental na cidade de Parnaíba, nordeste do Brasil. O objeto empírico foi composto por dez homens e dez mulheres com diagnóstico psiquiátrico. Adotou-se uma abordagem compreensiva e interpretativa, guiada principalmente pelos referenciais teóricos da fenomenologia social, do interacionismo simbólico, das teorias da ação estruturada e dos estudos feministas e de gênero. Elegeu-se enquanto método o estudo de casos e enquanto principal técnica de recolha de dados a entrevista de história de vida, visto que esta fornece elementos de como o social se manifesta através do “particular”. Também aplicou-se um pequeno questionário de caracterização sociodemográfica para perceber o peso de variáveis como status conjugal, idade, classe social, orientação sexual, raça/etnia, escolaridade, religiosidade. O tratamento e a análise das entrevistas foram realizados com base nas premissas da grounded theory. Complementarmente, recorreu-se à técnica de observação no interior do serviço, com registros em diário de campo, e a entrevistas semidiretivas com profissionais atuantes no CAPS II para entender melhor o contexto da cidade de Parnaíba quanto à reforma psiquiátrica. Os resultados revelam a pluralidade das experiências com uma doença mental. Se pertencimentos de gênero podem aproximar as experiências das mulheres entre si e dos homens entre si, a sua intersecionalidade com outros marcadores sociais pode, igualmente, produzir configurações extremamente diversas: ora aproximar as experiências das mulheres entre si, ora afastá-las ou, ainda, aproximar as suas experiências das de homens. Dito isto, analisou-se como relações desiguais de gênero aparecem nas explicações dos participantes a respeito da própria doença mental. Foram identificadas as concepções que os/as entrevistados/as têm sobre a sua própria condição, os “sintomas” psiquiátricos enunciados e os processos de (re)constituição dos percursos de adoecimento psíquico. Surgem categorias como: para mulheres e homens, o contexto familiar, as decepções amorosas, a pobreza e seus desdobramentos, ser alvo de bullying; para mulheres, especificamente, o exercício da sexualidade, a violência de gênero cometida por homens em suas relações interpessoais; para homens, especificamente, a violência de gênero entre homens, questões relacionadas com as expressões da orientação sexual e da identidade de gênero, vivência de aborto, vivência de HIV entre homens heterossexuais, o uso de álcool e/ou outras drogas. Viver com uma doença mental gera mudanças. Um primeiro impacto detectado é o estigma decorrente do rótulo psiquiátrico, que é potencializador de outras repercussões sobre a vida, pois aparece em detrimento de papéis, como os de trabalhador/a, pai, mãe, jovem, em várias situações sociais. Afeta a autoimagem e as interações em diversas esferas da vida cotidiana (a família, a vida laboral e os estudos, a sexualidade e as afetividades, as redes sociais). A despeito desses impactos, homens e mulheres mobilizam uma série de estratégias para lidar com a própria condição. Em conjunto, tais estratégias compõem os seus itinerários terapêuticos e sofrem influência de aspectos de gênero. Aqui, os itinerários terapêuticos abrangem estratégias de caráter formal (no sistema de saúde, seja pelo tratamento biomédico principalmente em instituições de caráter manicomial, seja pela frequência ao CAPS II) e informal (suporte familiar, de amigos, práticas variadas como atividades de cunho laboral ou criativo, lazer, espiritualidade/religiosidade). Desejos e planos para o futuro também são relatados. Percebe-se que, para a maioria dos sujeitos, tais desejos e planos envolvem a melhora da saúde. É a partir daí que poderão buscar concretizar ou retomar vivências outrora interrompidas pela doença mental. Muitas destas vivências são guiadas por uma ordem tradicional de gênero. Outras, acabam rompendo com tal ordem. A análise mostra diferenças significativas de gênero nas experiências de homens e mulheres com a própria doença mental. Adoecem de maneira distinta. Também sofrem de maneira diferenciada os impactos da doença, bem como mobilizam estratégias diversas para o seu enfrentamento. Os resultados reforçam o peso de aspectos sociais sobre a saúde mental, a valia na escuta das experiências subjetivas com a doença, a quebra de mitos (como o da fragilidade feminina ou da invulnerabilidade masculina) e a importância da pluralidade dos caminhos de gênero percorridos. As contribuições da pesquisa podem englobar usuários, gestores, trabalhadores, políticas de saúde mental.
The interface between gender and mental health has not yet been satisfactorily explored. Whether in terms of research or interventional practices, such interlinkages are insufficiently investigated, which causes unfavorable implications especially to people experiencing some psychic suffering. There is still little investment in this relationship beyond biomedical issues. It is therefore appropriate to deepen the knowledge of the ways in which gender is present in the experiences and meanings that mental illness has for those who suffer from it. Including the (inter) subjective dimension of the subjects is in accordance with practices driven by the psychiatric reform that have reshaped the model of mental health care in Brazil. Reformist principles and values have invested in the person, subjectivity, human interactions, experience with disease, therapeutic alternatives. Thus, we analyzed: what do people who experience some psychic suffering feel they have? What symptoms do they describe? What "causes" do they attribute to their condition? What changes in life do they perceive from the disease? What do they do to deal with disease and stigma? What expectations do they have about the future? What conceptions do they have about how other men and women may become ill? This research had a qualitative character and analyzed the men and women’s experiences diagnosed with mental illness, users of a Centro de Atenção Psicossocial II (CAPS II), mental health service in the city of Parnaíba, northeastern Brazil. The empirical object was composed of ten men and ten women with psychiatric diagnosis. A comprehensive and interpretative approach was adopted, guided mainly by the theoretical references of social phenomenology, symbolic interactionism, theories of structured action and feminist and gender studies in the interface with mental health. It was chosen as a method of case study and as the main research technique the interview of life story, since it provides elements of how the social manifests through the "particular." A sociodemographic characterization questionnaire was also applied to understand the weight of variables such as marital status, age, social class, sexual orientation, race / ethnicity, schooling, religiosity. The treatment and analysis of the interviews were based on the premises of grounded theory. Complementarily, we used the observation technique inside the service, field diary, and semi-directional interviews with professionals working in the CAPS II to better understand the psychiatric reform in Parnaíba. The results reveal the plurality of experiences with a mental illness. If gender “ties” can approximate women's experiences with each other, the intersectionality between gender and other social markers can also produce extremely diverse configurations: to hold off women's experiences with each other, to bring their experiences closer together of men. It was analyzed how unequal gender relations appear in the participants' explanations regarding their own mental illness. The interviewees' conceptions about their own condition, the psychiatric "symptoms" enunciated and the processes of (re) constitution of the pathways of psychic illness were identified. The following categories have been identified: for women and men, the family context, amorous disappointments, poverty and its consequences, bullying; for women, specifically, the exercise of sexuality, gender violence committed by men in their interpersonal relationships; for men, specifically, gender violence among men, issues related to expressions of sexual orientation and gender identity, experience of abortion, HIV experience among heterosexual men, use of alcohol and / or other drugs. Living with a mental illness causes changes. A first detected impact is the stigma arising from the psychiatric label, which is a potentiator of other repercussions on life, since it appears more than other roles, such as worker, father, mother, youth, in various social situations. It affects self-image and interactions in various spheres of daily life (family, working life and studies, sexuality and affectivity, social networks). Despite these impacts, men and women mobilize a number of strategies to deal with their condition. Taken together, these strategies make up their therapeutic itineraries and are influenced by gender aspects. The therapeutic itineraries cover formal strategies (in the health system, either by biomedical treatment mainly in asylum institutions, or by attendance at CAPS II) and informal strategies (family support, friends, varied practices such as activities work or creative, leisure, spirituality/religiosity). Desires and plans for the future are reported. It is perceived that, for the majority of the interviewees, such desires and plans involve the improvement of health. It is from there that they can seek to concretize or resume experiences once interrupted by mental illness. Many of these experiences are guided by a traditional gender order. Others break with that order. The analysis shows significant gender differences in the experiences of men and women with their own mental illness. They get sick differently. They also suffer in a different way the impacts of the mental illness, as well as mobilize diverse strategies for their confrontation. The results reinforce the weight of social aspects on mental health, the value in listening to subjective experiences with illness, the breakdown of myths (such as female fragility or male invulnerability) and the importance of the plurality of gender. Research contributions can encompass users, managers, workers, and mental health policies.
