Resumo(s)O pulso é uma importante região do corpo humano responsável por conectar o antebraço à mão, sendo essencial para a realização de inúmeras atividades básicas do dia-a-dia. A articulação do pulso (e antebraço) permite três graus de liberdade, responsáveis pelos movimentos de flexão-extensão, desvio radial-cubital e pronação-supinação. Existem inúmeras patologias que podem afetar o normal funcionamento do pulso, colocando-se especial enfoque nas tendinites, fraturas e doenças neurológicas. Assim, quando o pulso é lesionado, para além da dor que pode estar inerente, os seus movimentos também podem ficar comprometidos. O processo de reabilitação do pulso é baseado em sessões de fisioterapia, onde a sua mobilização é realizada através de movimentos ativos e passivos, com inclusão de dispositivos de reabilitação. Porém, estes não permitem todos os movimentos do pulso e antebraço, sendo necessária a sua substituição quando o movimento que conferem se encontra aprimorado. Além disso, não estão fortemente direcionados para os movimentos do pulso, encontrando-se mais focados no movimento de preensão. Os últimos desenvolvimentos efetuados baseiam-se em exosqueletos, realidades virtuais e terapias vibratórias para o auxílio da reabilitação do pulso. Nesta dissertação é proposto um novo dispositivo misto capaz de acompanhar o paciente em todo o seu processo de reabilitação, sendo a sua componente ativa do tipo vibratório e um dos focos deste estudo. Para este efeito foi desenvolvido um protótipo com forma cilíndrica (protótipo 1) e outro com forma de haltere (protótipo 2). A simulação numérica aponta que as frequências de ressonância dos dispositivos diminuem com o aumento do comprimento dos mesmos, sendo o dispositivo 2 o que apresenta frequências de ressonância mais baixas. Os resultados obtidos dos testes em pacientes evidenciaram que a terapia vibratória promove o alívio da dor e o aumento da amplitude articular, bem como a melhoria do movimento de preensão e da força, podendo ser usada logo numa fase precoce de reabilitação, tal como previsto pelos terapeutas. A par destes resultados, foi possível verificar que o sexo dos indivíduos influencia os resultados obtidos. Além disso, foi possível concluir que as vibrações que são transmitidas ao sistema mão-braço dependem da forma como o dispositivo é agarrado, influenciando o nível de exposição diária à vibração. As frequências naturais de ambos os protótipos não coincidem com as frequências de ressonância da mão e do braço, aspeto essencial para o sucesso da terapia. Embora as diferenças não sejam muito significativas, o protótipo 2 foi o que alcançou um maior ganho articular, reunindo assim aspetos fulcrais como pega ergonómica, forma atrativa e vibração adequada.
The wrist is an important region of the human body, that connects the forearm to the hand, being essential for the accomplishment of numerous basic daily activities. The wrist joint (and forearm) allows three degrees of freedom, responsible for flexion-extension, radial-ulnar deviation and pronation-supination movements. There are numerous pathologies that can affect the normal performance of the wrist, with a special focus on tendinitis, fractures and neurological diseases. Thus, when the wrist is injured, in addition to the inherent pain, its movements can also be compromised. The wrist rehabilitation process is based on physiotherapy sessions, where its mobilization is performed through active and passive movements, including rehabilitation devices. However, they do not allow all movements of the wrist and forearm, and their replacement is necessary when the movement that they confer is improved. In addition, they are not strongly directed to the wrist movements, being more focused on the grab movement. The latest developments are based on exoskeletons, virtual realities and vibratory therapies for the aid of wrist rehabilitation. In this thesis it is proposed a new mixed device able to accompany the patient throughout his rehabilitation process, being its active vibratory component and one of the focus of this study. For this purpose, a prototype with a cylindrical shape (prototype 1) and a prototype with a dumbbell shape (prototype 2) was developed. The numerical simulation shows that the resonant frequencies of the devices decrease with the increase of their length, presenting the second device the lowest resonance frequencies. The results obtained from the tests in patients showed that the vibratory therapy promotes the relief of the pain and the increase of the joint amplitudes, as well as the improvement of the grab movement and the strength, and can be used at an early stage of rehabilitation, as predicted by therapists. Moreover, it was possible to verify that the gender of individuals influences the results obtained. In addition, it was possible to conclude that the vibrations that are transmitted to the hand-arm system depend on the way the device is grabbed, influencing the level of daily exposure to vibration. The natural frequencies of both prototypes do not coincide with the resonance frequencies of the hand and arm, an essential aspect for the success of the therapy. Although the differences are not very significant, the prototype 2 was the one that achieved a greater joint gain, bringing together key aspects such as ergonomic handle, attractive shape and proper vibration.
