Resumo(s)The shoulder joint is the most movable joint of the human body. Due to its wide range of motion, the shoulder joint is also the most frequently dislocated joint in the human body. Once, a joint is dislocated its stability becomes affected which compromises its normal function, making it more difficult to realize the normal daily activities. The diagnosis of shoulder joint lesions is realized in two different steps: firstly, the physical exams, realized by the healthcare professional, with the aim of evaluating the joint function, through its palpation, as well as, the surrounding area. Moreover, in this exam, the motion of the joint is also simulated, moving it in different directions, in order to perceive abnormal motions of the joint. Secondly, the diagnosis is realized through an imaging exam, where is evaluated the position of the bones and, in case of some imaging exams, the condition of the structures which enfold the bones. The realization of the two aforementioned exams is performed in different instants in time conducting, sometimes, to divergent results, which can lead to an incorrect diagnosis of the patient condition. Furthermore, these diagnosis techniques do not allow the accuracy and the reproducibility of the results, since that, they rely on the subjective interpretation of the healthcare professional. Hence, in order to improve the quality of the diagnosis as well as to facilitate its process, the project design of a medical device to support the diagnosis of shoulder instabilities is proposed in the present dissertation. The development of the present project is divided in several stages, since the creation of a set of concepts to the medical device, to the validation of the final selected solution. Previously to the creation of concepts it was realized a survey to understand the customer needs. These were translated into functions, which device should realize, and a set of requirements, which medical device should contemplate. The concepts to the medical device were three-dimensional modulated to allow a clear comprehension of the respective functionality. The final solution was selected through a comparison evaluation between the concepts, using a set of selection criteria. Then, this final solution was improved, and it was validated. The final adopted solution possibilities the association, at the same time, of the shoulder joint movements with an imaging exam, like Magnetic Resonance Imaging. In this sense, the present medical device becomes the diagnosis more objective, allowing a more precise evaluation, which improves the quality of medical treatment.
A articulação do ombro é uma das articulações mais móveis do corpo humano. Devido á sua ampla gama de movimentos, esta articulação é também a articulação mais frequentemente deslocada do corpo humano. Uma vez que a articulação é deslocada, a sua estabilidade é afetada, dificultando a realização das atividades diárias. O diagnóstico de lesões na articulação do ombro é realizado em duas etapas distintas: primeiramente, é realizado um exame físico por um profissional de saúde, com o objetivo de avaliar a função da articulação, através da sua palpação, bem como da área envolvente. Além disso, neste exame são também simulados os movimentos da articulação, movendo-a em diferentes direções, de forma a percecionar movimentos anormais. Seguidamente, o diagnóstico é realizado através de um exame de imagem, onde é avaliada a posição dos ossos, e, no caso de alguns exames, a condição das estruturas que envolvem os ossos. Os dois exames são realizados em intervalos de tempo distintos, o que conduz, algumas vezes, a resultados divergentes, o que pode levar a um diagnóstico incorreto. Além disso, estas técnicas de diagnóstico não permitem a precisão e a reprodutibilidade dos resultados, uma vez que, dependem da interpretação subjetiva do profissional de saúde. Por isso, de forma a melhorar a qualidade do diagnóstico, bem como, facilitar o seu processo, apresenta-se neste trabalho, o projeto de design de um dispositivo para o apoio de diagnóstico de instabilidade da articulação do ombro. O desenvolvimento do presente projeto é dividido em diversas etapas, desde a criação de um conjunto de conceitos para o dispositivo, á validação da solução final. Inicialmente, foi realizado um estudo para perceber as necessidades dos consumidores. Estas necessidades foram traduzidas em funções, que o dispositivo deve realizar, e num conjunto de requisitos, que o dispositivo deve contemplar. Os conceitos foram modelados tridimensionalmente, para permitir uma melhor perceção da funcionalidade. A solução final foi eleita através da comparação entre conceitos, baseada num conjunto de critérios de seleção. De seguida, a solução final foi melhorada e validada. A solução final possibilita a associação, simultaneamente, dos movimentos da articulação, com um exame de imagem, como a Ressonância Magnética. Neste sentido, o presente dispositivo médico torna o diagnóstico mais objetivo, permitindo uma avaliação mais precisa, o que melhora a qualidade do tratamento médico.
