Resumo(s)No dizer da WHO (2000), os governos têm a responsabilidade de promover a saúde dos seus cidadãos. Para que possam cumprir este objectivo deverão assegurar que os profissionais de todos os sectores adquiram conhecimento apropriado, atitudes e competências para proteger e promover a saúde (WHO, 1999). Por outro lado, nenhum plano educacional deverá prescindir dos valores, uma vez que estes são os elementos que mais condicionam as escolhas das pessoas (Vallejo, 2002). De facto para que o processo de Promoção da Saúde (PrS) e de Educação para a Saúde (EpS) possa estar impregnado pelos valores da equidade e da participação, eles deverão ser adoptados pelos profissionais e futuros profissionais. Para isso os estabelecimentos de ensino deverão assumir este papel de educação para os valores devendo ainda identificar as concepções dos seus alunos. Assim, o objectivo geral do presente estudo é comparar o sistema de valores em PrS/EpS veiculados em cursos na área da saúde, ensino préescolar, ensino básico e serviço social, a fim de compreender a relação entre as concepções a ensinar e as concepções ensinadas. Para tal desenvolvemos um estudo descritivo, comparativo e transversal, cuja amostra é constituída por quatro docentes de Saúde Pública/Saúde Comunitária da Escola Superior de Enfermagem de Vila Real da Universidade de Trás-Os-Montes e Alto Douro (ESEVR-UTAD) e por 709 alunos de sete cursos do ensino superior. Como instrumentos de recolha de dados utilizamos a entrevista semi-estruturada realizada aos docentes e um questionário de auto-preenchimento aplicado aos alunos. Através da análise de conteúdo a modelos de PrS/EpS, em publicações nacionais e internacionais, identificamos um conjunto de seis eixos de valores em PrS/EpS: Social/ Individual (Soc/Ind), Salutogénico/Patogénico (Sal/Pat), Holístico/Reducionístico (Hol/Red), Equidade/Desigualdade (Equ/Des), Autonomia/Dependência (Aut/Dep) e Democrático/Autocrático (Dem/Aut), que constituem a matriz analítica do estudo. Comparando a perspectiva dos autores individuais e dos organismos oficiais com a perspectiva veiculada nos programas e documentos de ensino de Saúde Pública, verificamos que não existe concordância a nível da importância atribuída aos diferentes eixos de valores. No discurso dos autores individuais e organismos oficiais emergem mais os eixos da Aut/Dep, da Equ/Des e alguma preponderância é dada ao eixo Sal/Pat, no caso dos autores individuais, enquanto que nos programas do curso de Enfermagem de Vila Real (E-VR) e documentos de ensino a predominância do eixo Sal/Pat é esmagadora, sendo que nos programas o pólo Patogénico prevalece sobre o Salutogénico, o que nunca acontece no discurso dos autores individuais e organismos oficiais. O eixo Dem/Aut é sempre o menos emergente, existindo, neste caso, total sintonia. Na análise das entrevistas aos docentes da ESEVR-UTAD constatámos que os conceitos de Saúde são marcados pela positividade, subjectividade e holismo; os conceitos de doença assentam na objectividade; metade das entrevistadas expressou conceitos de Saúde Pública reducionistas; todas as entrevistadas têm uma perspectiva reducionista dos conceitos de Saúde Escolar e Saúde Infantil; o único conceito de PrS expresso é misto e embora tenham expressado facilmente o conceito de EpS metade das entrevistadas tem uma perspectiva passiva do conceito. Todas as entrevistadas concordam que os termos PrS e EpS são diferentes, mas uma delas não conseguiu efectuar a diferenciação. Todas concordam que os processos de PrS/EpS transmitem valores, tendo indicado 14 sub-categorias diferentes. Os valores considerados mais importantes foram a equidade, a solidariedade, a tolerância e o respeito. Todas demonstraram conhecer exemplos práticos relativos aos eixos Hol/Red e Dem/Aut. No que diz respeito às tendências das entrevistadas para um dos pólos dos eixos de valores, verificou-se estarem sempre na direcção dos pólos positivos. Responderam ao questionário 63,2% do universo dos alunos. No que se refere à representação mental do conceito de saúde, as cinco palavras-chave mais utilizadas pelo total da amostra foram (por ordem decrescente): Bem-estar, Hospital, Doença, Médicos e Enfermeiros, tendo a primeira sido utilizada por 77,4% dos alunos. A predominância destas palavras-chave poderá estar ligada a uma visão reducionista dos determinantes de saúde, centrada nas unidades de saúde, deixando de fora os outros determinantes de saúde. No que concerne à evolução do conceito de saúde, o curso de Enfermagem de Braga (E-BR) é onde se verificam mais descidas das palavras-chave ligadas à visão reducionista do termo e, da mesma forma, o curso de Professores do Ensino Básico (PEB) aquele onde existem mais aumentos ligados ao conceito abrangente. Pelo contrário, no curso de E-VR, não se notam alterações substanciais na perspectiva de saúde do 1º para o 4º ano, tendo ambos uma visão técnico-cêntrica da saúde. Do total da amostra, 86% dos alunos atribuem mais importância à perspectiva positiva de saúde, 88% à perspectiva dinâmica, enquanto que 54%, 91% e 65% elegem a perspectiva abrangente, respectivamente, de Saúde Pública, Saúde Escolar e PrS. A maioria dos alunos da amostra enquadra-se no conceito abrangente de PrS (51%) e no conceito activo de EpS (76%), considerando que os termos PrS e EpS são diferentes. A formação parece potenciar a construção de um conceito de PrS abrangente nos cursos de E-VR e Enfermagem do Porto (E-PO), nos quais se verificam diferenças altamente significativas (p= 0,000) do 1º para o 4º ano. A maioria da amostra (87%) consideram que a EpS tem valores a promover, tendo o conceito responsabilidade sido o mais indicado. Em relação à tendência dos alunos para os pólos dos eixos de valores, verificamos que a larga maioria tende para os pólos positivos. As maiores médias dos scores observaram-se no curso de E-BR e curso de Medicina (MED), enquanto que as menores no curso de Serviço Social (SSO). Os alunos do curso de E-VR obtiveram a média mais elevada na dimensão valoração e força, enquanto os alunos do curso de E-PO na dimensão actividade do conceito de EpS, sendo estas elevadas em todos os cursos. Os testes estatísticos mostraram ainda que a formação parece contribuir para o incremento destas dimensões. Apenas 41% dos alunos assinalaram ter realizado algum tipo de experiência prática, e destes todos indicaram realizar actividades de EpS, tendo o maior grupo assinalado que realiza práticas de EpS “Muitas vezes” (42%). A frequência das práticas aumenta do 1º para o 4º ano. A maior proporção de alunos realiza actividades na área de Saúde infantil (23%). Existem relações, embora ténues, entre concepções e conhecimentos, entre concepções e valores e entre concepções e práticas. Face a estes resultados, apresentamos uma proposta de linhas orientadoras para a formação inicial do curso de E-VR na área da PrS/EpS, que visa contribuir para a melhoria da qualidade de ensino e dos cuidados prestados à população.
According to WHO (2000), governments have the responsibility to promote their citizens’ health. In order to fulfil this aim they should guarantee that the professionals of all sectors acquire appropriate knowledge, attitudes and competences to protect and promote health (WHO, 1999). On the other hand, no educational plan should avoid values, since they are the elements that shape up most people’s choices (Vallejo, 2002). In order that the processes of Health Promotion (HP) and Health Education (HE) are impregnated with the values of equity and participation, they should be adopted by professionals and future professionals. For that, schools must assume this role of education for the values and should also identify the their students’ conceptions. Thus, the general aim of the present study is to compare the system of values in HP/HE conveyed in courses in the area of health, pre-school teaching, basic teaching and social service, in order to understand the relation between the conceptions to be taught and the taught conceptions. For this purpose, we developed a descriptive, comparative and cross-sectional study, which sample is composed of four teachers of Public Health/Community Health of the Vila Real Nursing College of the University of Trás-Os-Montes e Alto Douro (VRNC-UTAD) and by 709 students of seven university courses. The instruments to gather data were a semi-structured interview for the teachers and an auto-filling questionnaire for the students. Through the content analysis of HP/HE models in national and international publications, we identify a set of six axes of values in HP/HE: Social/Individual (Soc/ Ind), Saluthogenic/Pathogenic (Sal/Pat), Holistic/Reductionistic (Hol/Red), Equity/ Inequality (Equ/Ine), Autonomy/Dependence (Aut/Dep) and Democratic/Autocratic (Dem/Aut) which constitute the analytical matrix of the study. Comparing the perspective of the individual authors and of the official organisms with the perspective conveyed in the programs and teaching documents of PH, we verify that there is no agreement in what concerns to the importance attributed to the different axes of values. In the speech of the individual and official organisms the axes of Aut/Dep and Equ/Ine emerge more strongly and there is some preponderance of the axe Sal/Pat in the case of the individual authors, whereas in the programmes of the Vila Real Nursing course (VR-N) and respective teaching documents the predominance of the axe Sal/Pat is overwhelming. In these programmes the Pathogenic pole prevails over the Salutogenic one, which never happens in the speech of the individual authors and official organisms. The axe Dem/Aut is always the least emergent one and, in this case, there is total agreement. In the analysis of the interviews to the teachers of VRNC-UTAD we noticed that the concepts of Health are marked by positivity, subjectivity and holism; the concepts of disease are based in objectivity; half of the interviews expressed reductionist concepts of Public Health; all the interviews have a reductionist perspective of the Scholastic Health and Infantile Health concepts; the only expressed concept of HP is mixed and though they have easily expressed the concept of HE, half of the interviewees have a passive perspective of the concept. All the interviewees agree that the terms HP and HE are different, but one of them was not able to make the difference between them. All of them agree that the processes of HP/HE transmit values and they indicate 14 different sub-categories. The values that were considered more important were equity, solidarity, tolerance and respect. All of them demonstrated to know about practical examples concerning the axes Hol/Red and Dem/Aut. In what concerns the tendencies of the interviewees towards one of the poles of the axes of values, it was found that they are always in the direction of the positive poles. Of the students’ universe, 63,2% answered to the questionnaire. In what refers to the mental representation of the concept of health, the five key words used the most by the total of the sample were (by decreasing order): Well-being, Hospital, Disease, Doctors and Nurses, being that the first one was used by 77,4% of the students. The predominance of these key words can be connected to a reductionist vision of the determinants of health, centred in the system of health, excluding the other determinants of health. In what concerns the evolution of the concept of health, the Braga Nursing course (BR-N) is where we verified a higher reduction in the key words associated to the reductionist vision of the term and, similarly, the course of Basic Teaching Teachers (BTT) is where there are more increases associated to the wide concept of health. In contrast, the VR-N course did not show substantial changes on the perspective of health from the 1st to the 4th year, since both have a technical-centric view of health. Of the total students’ sample, 86% give more importance to the positive perspective of health, 88% to the dynamic perspective, while 54%, 91% and 65% elect the wide perspective of Public Health, School Health and HP, respectively. The majority of the students fits in the wide concept of HP (51%) and in the active concept of HE (76%), considering that the terms HP and HE are different concepts. Training seems to strengthen the construction of a wider HP concept in the VR-N and in the Porto Nursing course (PO-N), in which we verify highly significant differences (p=0,000) from the 1st to the 4th year. The majority of the sample (87%) considers that HE has values to promote, being the concept responsibility the most indicated one. In what concerns to the tendency of the students towards the poles of the axes of values, we verify that the majority aims at the positive poles. The scores highest means were found in the courses BR-N and Medicine (MED) whereas the smallest means were in the Social Service course (SSO). The students of the course VR-N obtained the higher means in the dimension valuation and strength whereas the students of the course PO-N obtained it in the dimension activity of the concept of HE, being these figures high in all courses. The statistical tests still showed that formation seems to contribute for the increase of these dimensions. Only 41% of the students marked to have carried out some type of practical experience and from these, all of them indicated to accomplish activities of HE, and the larger group marked that they do HE activities “Many times” (42%). The frequency of these activities increases from the 1st to the 4th year. The biggest ratio of students carries out activities in the area of Infantile Health (23%). There are associations, even though slender, between conceptions and knowledge, between conceptions and values and between conceptions and practices. Based on these results, we present a proposal of guidelines for the initial formation of the course VR-N on the area of HP/HE, that aims to contribute for the improvement of the quality of education and of the care offered to the population.
