Resumo(s)Tradicionalmente, a auto-revelação dos clientes é considerada como benéfica e um dos objectivos centrais da psicoterapia (Hill & O'Grady, 1985; Martin, 1984; Stiles, 1987), enquanto, no reverso, a ocultação tem sido interpretada como uma forma de resistência e consequentemente prejudicial. Ora, neste âmbito torna-se difícil integrar os resultados que denotam que os clientes, não obstante revelarem muito, também ocultam informação (cf. Hill, Thompson & Corbett, 1992; Kelly, 1998; Regan & Hill, 1992). Em relação aos terapeutas e ao processo de auto-revelação não existe igual consonância de perspectivas, tratando-se de um tema controverso (Hill & Knox, 2002). A ocultação dos terapeutas no encontro terapêutico tem sido ainda menos explorada, tanto no domínio teórico, como no domínio empírico. Estas discrepâncias e ambiguidades justificam a relevância do estudo deste tema. Assim, na parte teórica deste trabalho procurámos enquadrar os fenómenos em estudo, explorando a relação entre as diferentes abordagens terapêuticas e a auto-revelação/ocultação de terapeutas e clientes em psicoterapia, reflectindo acerca das mudanças no interior destas abordagens e dos desafios que se colocam aos terapeutas neste domínio. Seguidamente, procurámos apresentar e reflectir sobre a multiplicidade de variáveis estudadas em associação a estas práticas, assim como sobre o impacto destas no processo e no resultado terapêuticos. No plano empírico, o presente estudo centra-se na exploração de diferentes aspectos da experiência de ocultação em psicoterapia, inquirindo para o efeito díades de terapeutas e clientes sobre “não ditos” ocorridos durante a conversação terapêutica. Em termos da metodologia de investigação, optamos por uma metodologia exploratória, pelo que a recolha de dados privilegiou o relato dos intervenientes no processo terapêutico mediante a utilização de uma entrevista semi-estruturada, referente aos não ditos ocultados, aplicada a 11 díades em terapia individual, durante o decurso do processo terapêutico, nomeadamente na 4ª, na 8ª e na 12ª sessões. Os dados recolhidos foram analisados, seguindo os pressupostos e procedimentos da análise interpretativa fenomenológica (A.F.I.) (cf. Wertz, 2005), explorando os conteúdos e as funções dos não ditos. Entre as conclusões que retiramos dos resultados alcançados, destacamos a ocorrência frequente do fenómeno de ocultação no processo terapêutico, tanto na experiência de terapeutas como de clientes. Sintetizando os principais resultados do estudo qualitativo sobre os conteúdos dos não ditos, constatamos a predominância da ocultação de conteúdos negativos por parte de ambos os participantes. Em relação aos terapeutas, estes ocultam significativamente mais conteúdos relacionados com as dificuldades percebidas nos clientes, e no caso destes, assume destaque a ocultação de preocupações ou dificuldades com aspectos processuais. Quanto às motivações para recorrer à ocultação, encontramos motivos caracterizados como Reflexivos e Estratégicos. No domínio Reflexivo, os resultados evidenciam o predomínio da ocultação dos terapeutas por motivações associadas ao Terapeuta. Igualmente, dado o interesse em compreender o impacto destes fenómenos em termos do processo e do resultado da psicoterapia, procurámos formar e contrastar grupos e casos com resultados divergentes (bom resultado/resultado pobre) no processo terapêutico. Partindo da reflexão acerca do significado destas categorizações e da sua evolução ao longo do processo terapêutico, procurámos reflectir sobre os principais desafios que se colocam actualmente na prática clínica e na formação e supervisão de terapeutas neste âmbito.
Traditionally, clients’ self-disclosure is considered beneficial and one of the central aims of the psychotherapeutic process (Hill & O'Grady, 1985; Martin, 1984; Stiles, 1987), while, in the reverse, clients concealment has been interpreted as a form of resistance and therefore detrimental. However, in this scope it’s difficult to integrate the empirical results that show that clients, although being self-disclosing, also conceal information (cf. Hill, Thompson & Corbett, 1992; Kelly, 1998; Regan & Hill, 1992). In relation to the therapists and the process of self-disclosure we didn’t found equal accord of perspectives, remaining a controversial subject (Hill & Knox, 2002). Therapists’ concealment remains even less explored, as much in the theoretical as in the empirical domain. These discrepancies and ambiguities justify the importance of the study of this subject. Thus, in the theoretical part of this work we tried to grasp the phenomenon in study, exploring the relation between the different therapeutic approaches and therapists’ and clients’ self-disclosure and concealment in psychotherapy, pondering about the main changes in the realm of those approaches and about the challenges that therapists face in this domain. Next, we tried to look at the multiplicity of dimensions that have being studied regarding these practices, as well as to its’ impact in the psychotherapy process and outcome. In the empirical plan, the current study focus on the examination of diverse aspects of the concealment experience in the therapeutic process, by means of inquiring therapists and clients counselling dyads about the occurrence and effects of those “things left unsaid” in the therapeutic conversation. In terms of the inquiry methodology, we chose an exploratory methodology, so in terms of the data collection we privileged the reports of the participants in psychotherapy, by means of using a semi-structured interview concerning the things left unsaid. We applied this interview to eleven individual psychotherapy dyads, during the course of the therapeutic process, namely at the 4th, 8th and 11th sessions. The data that where collected where analysed accordingly to the principles and procedures of the Interpretative Phenomenological Analyses (I.P.A.) (cf. Wertz, 2005), exploring both the contents and functions of those things left unsaid. Among the conclusions that we reached, we would like to point the elevated occurrence of the concealment phenomenon in the psychotherapeutic process, both in the clients’ and in therapists’ experience. Summarizing the main results of the qualitative study about the contents of the things left unsaid, we point to the predominance of the concealment of negative contents on the part of both participants. In relation to the therapists, they concealed significantly more contents related with the difficulties perceived in the clients, and in the case of these, it assumes prominence the concealment of concerns or difficulties with procedural aspects. With reference to the reasons to use self-concealment, we found reasons characterized as Reflective and Strategic. In the Reflective domain, the results support the predominance of the therapists’ concealment for motivations associates to the Therapist. Equally, given the interest in understanding the impact of this phenomenon in terms of the psychotherapy process and outcome, we tried to delineate and contrast divergent groups and cases in terms of the therapeutic outcome (good and poor outcome). Based on our reflections concerning the meaning of this categorization and its evolution throughout the therapeutic process, we tried to consider the main challenges that these results currently place, both in terms of clinical practice and in the context of therapists’ formation and supervision.
