Resumo(s): | A vasta investigação relativa à relação terapêutica é reveladora da sua importância para
a obtenção do sucesso terapêutico (Horvath & Symonds, 1991; Horvath & Bedi, 2002;
Kazdin, 2007). A Aliança Terapêutica apresenta-se como uma dimensão primordial da
relação terapêutica. Uma Aliança Terapêutica positiva, de qualidade, é propiciadora de
resultados terapêuticos igualmente positivos (Horvath & Bedi, 2002; Horvath & Greenberg,
1994; Horvath & Symons em 1991; Luborsky, 1994; Martin, Garske, Davis, 2000; Tryon &
Winograd, 2001, 2002).
Segundo a conceção de Bordin (1979), a Aliança Terapêutica é entendida como fator
relevante na obtenção de uma relação terapêutica bidirecional, ou seja, numa relação
terapêutica assente na colaboração mútua, marcada pela negociação de objetivos e tarefas
entre o terapeuta e o cliente. Partindo do conceito de colaboração mútua associado a esta
definição de aliança Ribeiro et al. (2011) procuraram contribuir para responder à questão de
como a aliança se torna terapêutica (Horvath, 2006). Os autores desenvolveram uma
proposta teórica e metodológica no sentido de compreender como se desenvolve o
processo de colaboração entre o terapeuta e o cliente e de que modo este processo se
articula com a mudança em terapia. Neste sentido, Ribeiro et al. (2011) utilizaram o conceito
de Zona Desenvolvimental Proximal Terapêutica - ZDPT proposta por Leiman & Stiles
(2001). Entendida como a distância entre o desenvolvimento atual do cliente e o nível de
desenvolvimento potencial que este poderá atingir em colaboração com o terapeuta.
É no âmbito do trabalho de Ribeiro et al. (2011) que se enquadra esta investigação. Mais
especificamente, esta investigação visa caracterizar a colaboração terapêutica na última
sessão de casos Dropout, seguidos em terapia cognitivo-comportamental. A amostra deste
estudo é constituída por 9 últimas sessões de casos de Dropout, distribuindo-se estas
últimas sessões entre a 3ª e 11ª sessão. Por via da utilização do Sistema de Codificação da
Colaboração Terapêutica, criado por Ribeiro et al. (2010), obteve-se a frequência dos
episódios interativos que ocorreram dentro, no limite e fora da ZDPT. Através da
metodologia do State Space Grids (Lamey, Lewis & Granic, 2004) obtiveram-se os episódios
que correspondiam a mais de 80% das interações de cada sessão anterior ao Dropout. Os
resultados revelaram que o mais frequente é o terapeuta trabalhar à frente do Cliente na
ZDPT, ou seja, o terapeuta tende a desafiar o cliente a inovar, sendo que o cliente se recusa
a sair da sua experiência de segurança, tendendo a ficar mais próximo do seu estado atual. The wide research concerning to the therapeutic relationship is revealing of its value to obtain therapeutic sucesses (Horvath & Symonds, 1991; Horvath & Bedi, 2002; Kazdin, 2007). The Therapeutic Alliance is presented as being a primordial dimension of the therapeutic relationship: A positive and quality Therapeutic Alliance leads to likely positive therapeutic results.( Horvath & Bedi, 2002; Horvath & Greenberg, 1994; Horvath & Symons em 1991; Luborsky, 1994; Martin, Garske, Davis, 2000; Tryon & Winograd, 2001, 2002). According to Bordin´s (1979) theory, the Therapeutic Alliance is understood as a revealing factor to obtain a bidirectional therapeutic relationship, ie, a therapeutic relationship based on mutual cooperation, standed by the negotiation of tasks and goals between therapist and patient. Starting from the concept of mutual cooperation combined with the definition of alliance, Ribeiro et al. (2011) sought to help answering about how the alliance becomes therapeutic (Horvath, 2006). The authors have develop a theoretical and methodological proposal in order to understand how the development of the collaboration process between therapist and patient occurs, and at the same time, in which ways this process is articulated with the change of therapy. Therefore, Ribeiro et al. (2011) used the "Therapeutic Proximal Development Zone" - ZDPT, proposed by Leiman & Stiles (2011), known as being the distance between the patient present development and the level of probable development which the patient might reach collaborating with the therapist. This investigation is based on Ribeiro et al.(2011) work, more specifically, it tries to characterize the therapeutic collaboration during the last session of Dropout cases, followed with behavioural and cognitive therapy. The sample of this study consists in the last 9 sessions of Dropout cases, distributing these between the 3rd and 11th sessions. Using the therapeutic collaboration coding system, created by Ribeiro et al. (2010), we have reached to the frequency of the interactive episodes whom occur within, at the limit and out the ZDPT. Through the "State Space Grids" methodology (Lamey, Lewis & Granic, 2004) we came to the episodes that match in over 80% of the interactions of every previous Dropout session. The results revealed that what frequently happens is that the therapist works ahead of the patient on the ZDPT, ie, the therapist tends to outdare the patient to innovate, leading the patient to refuse abandoning his own secure experiment tending to keep closer to his current condition. |
