Resumo(s): | A longevidade, traço significativo na evolução das sociedades actuais,
conquistada não só através dos progressos científicos mas também das
melhores condições sociais e económicas, suscita questões que se cruzam
entre o viver mais tempo, a ocorrência de maior número de patologias e a
qualidade de vida.
O Acidente Vascular Cerebral, apesar de poder ocorrer em qualquer
idade, tem maior incidência em populações idosas. Nestas, a sua ocorrência
pode ter efeitos especialmente devastadores, em particular, se existem outros
factores de risco como a presença de outras patologias, a ausência de suporte
familiar e social ou ainda os fracos recursos económicos, podendo alterar
significativamente a vida do doente e repercutindo-se igualmente na sua
qualidade de vida, na do seu cuidador informal e na da sua família.
O estudo de carácter longitudinal que desenvolvemos junto de doentes
que sofreram um AVC e respectivos cuidadores informais, referenciados pelo
Centro de Saúde de Alfândega da Fé, procurou caracterizar os doentes sob o
ponto de vista sócio-demográfico e da prevalência de deficiências e
(in)dependência funcional; conhecer os cuidadores informais sob o ponto de
vista sócio-demográfico; analisar a qualidade de vida e as repercussões
familiares e sociais nos doentes e nos seus cuidadores informais; estudar a
satisfação em relação aos serviços sociais e de saúde por parte dos doentes
que sofreram o AVC e dos seus cuidadores informais; analisar a relação entre
a qualidade de vida do cuidador e a (in)dependência funcional do doente,
considerando os seus géneros e os seus rendimentos mensais.
Os resultados obtidos revelam que se trata em maioria de uma
população idosa, pouco escolarizada, pertencendo a baixos estatutos sócioprofissionais,
casada, sendo que 57,1% vivem apenas com o seu cônjuge. Os efeitos do AVC repercutiram-se a nível pessoal, traduzido nos défices
neurológicos, mas também nas alterações na vida conjugal, familiar – ainda
que os laços familiares sejam reforçados – e social, muito concretamente na
diminuição da participação em actividades sociais e recreativas e nos
relacionamentos com amigos. A nível económico é clara a perda de rendimento
mensal acentuando-se ainda com o aumento das despesas decorrentes das
necessidades desencadeadas pela doença.
Os cuidadores informais são predominantemente mulheres com uma
média de idades de 62,9 anos, baixa escolaridade, empregos pouco
especializados e baixos rendimentos mensais. A saúde dos cuidadores
deteriorou-se, assumindo “que cuidar de si passou para segundo plano”. Mais
de 70% afirmaram que o AVC provocou transtornos na família mas, e apesar
disso, as ligações entre a família e amigos saíram reforçadas.
No nosso trabalho, o facto de ser marido/mulher ou filha, foram variáveis
determinantes para assumir o papel de cuidador, sendo que os papéis dos
cuidadores informais são grandemente influenciados pelas normas sociais e
culturais. Longevity, a significant feature in the evolution of modern society, achieved not only through scientific progress but also through better social and economic conditions, raises issues that come from living longer, the occurrence of a greater number of diseases and the quality of life. The stroke, although it can occur at any age, has a higher incidence in older populations. Accordingly, its occurrence may have particularly devastating effects, specially if other risk factors exist, such as the presence of other diseases, lack of family support or poor economic resources, and can significantly alter the patient's life, the quality of life of their informal caregivers and their families. The longitudinal study of character developed on patients who suffered a stroke and their informal caregivers, referenced by the Health Centre of Alfândega da Fé, aimed to characterize patients from the point of view of sociodemographic conditions and the prevalence of disability and functional (in)dependence; study the informal caregivers from a socio-demographic point of view; analyze the quality of life and the family and social consequences for patients and their caregivers; study the satisfaction with health and social services by patients who suffered a stroke and their caregivers; analyze the relationship between the caregiver's quality of life and the functional (in)dependence of the patient, considering their gender and their monthly income. The results show that it is mostly an elderly population, with little schooling, belonging to low socio-professional status, married, and that 57.1% live only with their spouse. The effects of stroke affected them on a personal level, and manifested not only as neurological deficits, but also as changes in marital and family life - even when family ties were strengthened - and in social life, specifically in a decreased participation in social and recreational activities and in the relationships with friends. Economically there is a clear loss of x monthly income and an increase of the expenses related with the needs caused by the disease. Informal caregivers are predominantly women with an average age of 62.9, low education, low-skilled jobs and low monthly income. The caregivers' health deteriorated, assuming "that caring for themselves became secondary”. Over 70% said that the stroke caused problems in the family but, despite this, the links between family and friends come out strengthened. In this work, the fact of being a husband/wife or daughter, were determinant variables to assume the caregiver role, the role of informal caregivers being greatly influenced by social and cultural norms. |
