Resumo(s)Os ganhos súbitos consistem numa mudança sintomática abrupta significativa entre duas sessões consecutivas. Diversos estudos analisaram o impacto dos ganhos súbitos no resultado terapêutico e no follow-up, mas poucos investigaram os mecanismos associados a este fenómeno. Os momentos de inovação (MIs) e os marcadores de ambivalência (MAs) são variáveis processuais que se têm demonstrado preditoras do sucesso terapêutico. Objetivo: O principal objetivo deste estudo foi analisar a emergência de MIs e MAs nos ganhos súbitos. Método: A amostra foi composta por 26 clientes de psicoterapia com perturbações emocionais, acompanhados com o Protocolo Unificado para o Tratamento Transdiagnóstico de Perturbações Emocionais. Resultados: Os clientes com ganhos súbitos apresentaram uma melhoria sintomática mais significativa no final do tratamento, comparativamente aos clientes sem ganhos súbitos. Análises multinível sugerem uma diminuição dos MIs de nível 1 e uma tendência decrescente de MAs mais pronunciada nos clientes que experienciaram ganhos súbitos. Adicionalmente, estes clientes apresentaram maior emergência de MIs de nível 2 após o ganho súbito. Discussão: De acordo com os resultados do estudo, os ganhos súbitos parecem estar associados a uma maior integração da mudança e a uma resolução da ambivalência mais rápida.
Sudden gains consist of a significant abrupt change of symptoms between two consecutive sessions. Several studies have analysed the impact of sudden gains on outcome, as well as follow-up, but few focuses on the mechanisms associated with this phenomenon. The Innovative Moments (IMs) and the Ambivalence Markers (AMs) are procedural variables that have been shown to be predictors of therapeutic success. Objective: The main purpose of this study was to analyse the emergence of IMs and AMs in sudden gains. Method: The sample consisted of 26 psychotherapy outpatients with emotional disorders, followed with the Unified Protocol for the Transdiagnostic Treatment of Emotional Disorders. Results: Sudden gainers showed a more significant symptomatic improvement at the end of the treatment when compared to non-sudden gainers. Multilevel analysis suggest a decrease in level 1 IMs in sudden gainers and a more pronounced downward trend in ambivalence in clients who experienced sudden gains. Besides, sudden gainers showed an increase of level 2 IMs after the sudden gain. Discussion: According to the results of the study, sudden gains appear to be associated with a greater integration of change and a faster ambivalence resolution.
