Colaboração terapêutica em episódios de ambivalência

Abstract

A ambivalência é parte integrante do processo de mudança em psicoterapia, mas a sua resolução assume um papel fulcral, para a qual colaboração terapêutica tem sido associada à evolução positiva dos resultados. O objetivo foi analisar de modo compreensivo a intervenção terapêutica e a resposta de duas clientes com diferentes resultados terapêuticos, nos episódios de ambivalência e da sua resolução, bem como o trabalho terapêutico na Zona Terapêutica de Desenvolvimento Proximal. Usando uma metodologia de estudo de caso aplicamos o TCCS (Ribeiro et al., 2013) a dois casos clínicos, de sucesso e insucesso, seguidos em CBT que expressaram ambivalência. Os resultados sugerem que o terapeuta parece intervir de forma semelhante em ambos os casos, mas os clientes respondem de forma diferente: o caso de sucesso responde no mesmo nível de desenvolvimento ou acima do nível proposto, ao passo que o caso de insucesso trabalha no mesmo nível ou aquém. Este estudo é consistente com a teoria de que nos casos de insucesso o trabalho terapêutico ocorre à frente da ZTDP, e além disso, o nível de preparação para a mudança, a motivação, e o compromisso são fundamentais para o sucesso terapêutico.

Ambivalence is part of the process of change in psychotherapy, but its resolution plays a key role, for which therapeutic collaboration has been linked to positive development of results. The objective was to analyze in a comprehensive manner the therapeutic intervention and the response of two clients with different therapeutic results in episodes of ambivalence and its resolution, and the therapeutic work in the therapeutic Zone of Proximal Development. Using a case study methodology, we applied TCCS (Ribeiro et al, 2013) to two CBT clinic cases with one successful and one unsuccessful. The results suggest that the therapist tended to intervene in a similar way in both, but the clients responded in a different way: the successful case worked in same developmental level proposed or above, whereas the unsuccessful case responded in the same level or below. This study is consistent with theory that in unsuccessful case the therapeutic dyad worked above the current TZPD, contributing to the maintenance of ambivalence and moreover, the level of preparation for change, motivation, and commitment are fundamental to therapeutic success.
