Resumo(s): | Acute myeloid leukemia (AML) is the most common type of acute leukemia in adults, being often
associated with poor outcomes. Such dismal prospect has not changed over the past decades and
the therapeutic strategies have also not been significantly modified over the last 30 years. Although
the therapeutic regimens result in positive responses in young patients, intensified therapy is highly
toxic and has limited applicability among old individuals, the most affected population, resulting in
high mortality rates. Intriguingly, the survival and chemoresistance mechanisms of AML cells are
still elusive. Understanding the pathogenetic mechanisms underlying this malignancy has therefore
critical relevance.
Mounting evidence has shed light on the relevance of the two main proteolytic systems (autophagy
and ubiquitin-proteasome system – UPS), as well as the AMPK (AMP activated Protein Kinase)
pathway, on AML biology and treatment, but their exact role is still elusive. Therefore, the main aim
of this thesis was to obtain new insights on the contribution of the proteolytic systems (and
upstream regulators) on AML prognosis, development and response to antileukemia therapy. Using
two AML cell lines, HL-60 and KG-1, representative of two different AML subtypes, the antileukemia
agents were shown to target both proteolytic systems, as well as, the AMPK pathway. In addition,
an heterogeneous response of different AML subtypes was observed to the broadly used
therapeutic scheme, cytarabine and/or doxorubicin, being clear that autophagy and UPS may play
a protective role depending on the AML subtype. The modulation of the proteolytic systems is,
therefore, viewed as a promising novel treatment option.
The presence of metabolic disorders, including type 2 diabetes mellitus (DM), obesity and high
body mass index (BMI), has been associated with the occurrence and poor prognosis of AML.
Evidence showed that both type 2 DM and obesity are associated with increased AML incidence.
However, the biological mechanisms underlying these associations remain also unclear. In the
same context, autophagy and UPS are also known to be profoundly affected by the individual
metabolic state promoted by several metabolic disorders, including the previous mentioned.
Therefore, the present thesis also aimed to clarify the association between the presence of
metabolic disorders and the development and prognosis of AML, taking into account the proteolytic
systems functioning. Using clinical data of 164 AML patients diagnosed and treated at IPO-Porto,
the results achieved support the hypothesis that type 2 DM is associated with a diminished overall
survival (OS) and relapse free survival (RFS) in AML patients, while the presence of abnormal BMI levels (defined as ≥25Kg/m2) did not affect the same endpoint variables. Moreover, mRNA
expression of molecules involved in the proteolytic systems and their regulation is lower in bone
marrow (BM) samples of AML patients, when compared with the cohort without hematological
malignancies. Furthermore, the mRNA levels of these molecules are even more reduced in AML
patients with type 2 DM. Overweight does not seem to affect the proteolytic systems levels in AML
patients. Furthermore, our results suggested that the BCL2 gene is an outcome predictor for AML.
Thus, these results highlight once again that the proteolytic systems are crucial players on AML
pathology.
An association between AML and single nucleotide polymorphisms (SNPs) in central players of the
proteolytic systems and AMPK pathway was also assessed during this thesis. Our data showed that
the SNP rs1864182, in ATG10 gene, and the SNP rs3738568, in AMPKα2 gene, are associated
with AML. Concerning the functional consequences of these variants, the SNP rs1864182 in
ATG10 gene seems to cause a reduction of LC3 protein levels. Moreover, the SNP rs3738568 in
AMPKα2 gene caused a marked reduction of the total and phosphorylated AMPK protein levels.
Altogether, our findings suggest that AMPK and the proteolytic systems play an important role in
AML progression and tumorigenicity, as well as in the response to conventional chemotherapy
schemes. Moreover, our results disclose, for the first time, the association between genetic
variations in ATG10 and AMPK genes, and AML, highlighting that the processes where these genes
participate are crucial for this disorder. Consequently, it is imperative to uncover the biological
relevance of these SNPs on the outcome of AML, through functional studies further detailed. These
findings open new research avenues to the study of the mechanisms underlying AML pathogenesis. A leucemia mielóide aguda (LMA) é o tipo mais comum de leucemia aguda em adultos, porém é a leucemia que apresenta menor taxa de sobrevivência. Esta perspetiva não se alterou nas últimas décadas e a estratégia terapêutica aplicada nas LMA não tem sofrido mudanças significativas ao longo dos últimos 30 anos. Embora estes regimes terapêuticos apresentem efeitos positivos em pacientes jovens, a terapia intensiva é altamente tóxica, apresentando uma aplicação limitada e ineficaz entre os indivíduos mais velhos, a população mais afetada, resultando assim em altas taxas de mortalidade. Curiosamente, os mecanismos de sobrevivência e resistência de células de LMA ainda são desconhecidos. Compreender os mecanismos patogénicos desta neoplasia tornase, portanto, fundamental. Enumeras evidências têm demonstrado a relevância dos dois principais sistemas proteolíticos (autofagia e sistema ubiquitina-proteassoma (proteassoma)), bem como da via AMPK (AMP activated Protein Kinase), na biologia e tratamento da LMA, porém o papel exato destes mecanismos ainda se encontra por compreender. Desta forma, o principal objetivo deste trabalho caracterizou-se pela obtenção de novos conhecimentos acerca do contributo dos sistemas proteolíticos (e seus reguladores) no prognóstico, desenvolvimento e resposta à terapia da LMA. Utilizando duas linhas celulares de LMA, HL-60 e KG-1, representativas de dois subtipos distintos desta doença, verificou-se que os agentes usados na terapia anti-leucémica atingem os dois sistemas proteolíticos, assim como a via do AMPK. Além disso, foi observada uma resposta heterogénea por parte dos dois subtipos de LMA ao esquema terapêutico utilizado, sendo evidente que a autofagia e o proteassoma podem desempenhar um papel protetor, dependendo do subtipo de LMA. A modulação dos sistemas proteolíticos é, assim, considerada uma estratégia de tratamento promissora. A presença de doenças metabólicas, incluindo diabetes mellitus (DM) tipo 2, obesidade e elevado índice de massa corporal (IMC), tem sido associada à ocorrência e a baixo prognóstico da LMA. Evidências mostraram que a DM tipo 2 e a obesidade estão associadas com uma maior incidência de LMA. No entanto, os mecanismos biológicos subjacentes a esta associação permanecem por clarificar. Neste mesmo contexto, a autofagia e o proteassoma são também reconhecidos por serem profundamente afetados pelo estado metabólico de cada indivíduo, promovido por diversos distúrbios metabólicos como os anteriormente mencionados. Assim, nesta tese também se pretendeu clarificar a associação entre a presença de doenças metabólicas e o desenvolvimento e mau prognóstico da LMA, tendo em conta a atividade/funcionamento dos sistemas proteolíticos. Os resultados obtidos, utilizando dados clínicos de 164 pacientes com LMA, diagnosticados e tratados no IPO-Porto, apoiam a hipótese de que a DM tipo 2 está associada a uma menor sobrevida global, bem como a uma menor sobrevida livre de recaída nestes pacientes. Por sua vez, a presença de níveis anormais de IMC (≥25Kg/m2) não afetou as mesmas variáveis. Adicionalmente, a expressão de mRNA dos diferentes reguladores dos sistemas proteolíticos é menor nas amostras de medula óssea (MO) de pacientes com LMA, quando comparada com a coorte sem doenças hematológicas. Além disso, os níveis de mRNA destes reguladores ainda se encontram mais reduzidos em pacientes com LMA e com DM tipo 2. O excesso de peso não parece afetar os níveis de expressão dos sistemas proteolíticas em pacientes com LMA. Adicionalmente, os nossos resultados sugerem que o gene BCL2 é um preditor de outcome para a LMA. Assim, estes resultados destacam, uma vez mais, que os sistemas proteolíticos são intervenientes cruciais na LMA. Uma associação entre LMA e polimorfismos em reguladores centrais dos sistemas proteolíticas e da via do AMPK foi também estudada durante esta tese. Os nossos dados mostraram que o polimorfismo rs1864182 no gene ATG10 e o polimorfismo rs3738568 no gene AMPKα2 estão associados a LMA. Em relação às consequências funcionais destas variantes, o polimorfismo rs1864182 no gene ATG10 parece causar uma redução nos níveis proteicos de LC3. Além disso, o polimorfismo rs3738568 no gene AMPKα2, causou uma redução evidente dos níveis totais e fosforilados da proteína AMPK. Em conclusão, os nossos resultados sugerem que a via do AMPK e os sistemas proteolíticos desempenham um papel importante na progressão e tumorigenicidade da LMA, bem como na resposta à quimioterapia convencional. Além disso, os nossos dados mostram, pela primeira vez, uma associação entre as variantes genéticas nos genes ATG10 e AMPK e a LMA, destacando que os processos onde estes genes participam são cruciais para esta doença. Assim, torna-se imperativo descobrir a relevância biológica destes polimorfismos na LMA, realizando estudos funcionais mais detalhados. Estes resultados abrem novas perspetivas para o estudo de mecanismos associados com a patogénese da LMA. |
