Validação do Eating Disorder Examination Questionnaire (EDE-Q) para a população bariátrica portuguesa

Abstract

O Eating Disorder Examination-Questionnaire (EDE-Q) tem sido o instrumento mais utilizado para identificar patologias de Perturbações do Comportamento Alimentar, mas o uso na população bariátrica ainda é limitado. Este estudo pretende analisar e comparar as propriedades psicométricas das versões EDE-Q com 28, 7, 8 e 18 itens, em 262 participantes avaliados nos momentos pré-cirurgia, 12 meses e 24 meses após cirurgia bariátrica. Os participantes responderam ao EDE-Q28, a outras medidas de psicopatologias alimentares e de comorbilidades associadas (ansiedade, depressão e stress). As versões EDE-Q apresentaram bons valores de consistência interna (α > .71). Na validade convergente, as versões EDE-Q28 e EDE-Q8 obtiveram correlações mais altas com as outras medidas, nos três momentos. Na validade divergente, as versões EDE-Q com 28 e 8 itens conseguiram diferenciar entre participantes com e sem episódios de comportamentos alimentares problemáticos. Os resultados da Análise Fatorial Confirmatória mostraram bom ajustamento das versões, excetuando no EDE-Q8. Observou-se sensibilidade à mudança das versões EDE-Q nos momentos pré–12 meses após cirurgia (F (3, 783) = 37.68, p < .001), com o EDE-Q8 a demonstrar maior sensibilidade. Concluindo, aconselha-se a aplicação da versão EDE-Q28, podendo utilizar-se o EDE-Q8 como medida de triagem ou para detetar mudanças durante o tratamento.

The Eating Disorder Examination-Questionnaire (EDE-Q) is a widely used self-report measure to identify pathologies in eating disorders, but its use in the bariatric population is still limited. This study aims to analyze and compare the psychometric properties of EDE-Q versions with 28, 7, 8 and 18 items, in 262 participants evaluated in the pre-surgery, 12- and 24-months follow-up. Participants completed the EDE-Q28, other measures assessing disordered eating and associated psychopathologies (anxiety, depression, and stress). The EDE-Q versions showed good values for internal consistency (α > .71). In testing convergent validity, the EDE-Q28 obtained higher correlations in the three moments evaluated, and in the discriminant validity, the EDE-Q versions with 28 and 8 items managed to differentiate participants who reported/didn’t report episodes of problematic eating behaviors. Confirmatory Factor Analysis failed to support the factor structure of EDE-Q8 and accepted the other three versions (28, 7, and 18). Sensitivity to change was observed in the pre-surgery/12 months follow-up (F (3, 783) = 37.68, p < .001), with EDE-Q8 having greater sensitivity to change. In conclusion, it is advisable to apply the EDE-Q with 28 items, and the EDE-Q8 can be used as a screening measure or to detect changes across treatment.
