Research on the therapeutic alliance (TA) in eating disorders has been directed towards its association with therapeutic outcomes, which encourages the exploration of TA predictors. Therefore, this study aims to clarify the role of TA and early change in therapeutic outcomes, as well as explore the potential predictors of the therapeutic alliance. The participants were 51 individuals (mostly females) diagnosed with an eating disorder, undergoing specialized treatment for eating disorders (AP). In short, AT was a significant predictor of the subsequent change in eating restriction. Early change was not a predictor of any type of subsequent change in food pathology and, together with AT, were not significant predictors of clinical status at the end of treatment. Only the BMI, the time of evolution of the disorder and the frequency of objective bulimic episodes predict the AT. The results refer to the importance of AT in the first weeks of treatment, instead of behavioral change, namely in anorexia nervosa, as well as to the consideration of certain patient characteristics for treatment success.
