The use of scleral lenses (SL) has increased significantly in recent years. With the expansion of their clinical indications, questions have arisen regarding their impact on intraocular pressure (IOP), since their design and scleral support may hypothetically interfere with aqueous humour drainage. The main limitation is the lack of effective methods to measure IOP without removing the lens. Therefore, it is essential to develop ways to monitor IOP with the lens in situ. The present study aimed to evaluate the feasibility of measuring IOP during SL wear. For this purpose, a 1,00 mm fenestration (in the central or temporal zone) was made, and a rebound tonometer (iCare, Helsinki, Finland) with a modified probe was used. 14 healthy adults (mean age: 23,86 ± 3,18 years) participated and were evaluated in three distinct visits: (1) reference value (baseline, without lens); (2) and (3) IOP was measured before insertion, immediately after, after 90 minutes of wear, and after removal of the Hexafocon A SL (Senso MiniSclera, Procornea, Netherlands), using identical parameters differing only in fenestration location. Additional parameters evaluated included fluid reservoir thickness (FRT), central corneal thickness (CCT) measured using anterior optical coherence tomography (OCT), ocular surface health (slit lamp), and the comparison between the two tonometers (iCare vs. ORA). During the visit without lenses, a significant reduction in temporal IOP was observed after 90 minutes (-1,55 mmHg; p = 0,026), while central IOP remained unchanged. With the centrally fenestrated lens, central IOP increased after 90 minutes (+1,69 mmHg; p = 0,061), returning to initial values after lens removal. With the temporally fenestrated lens, temporal IOP increased (+2,40 mmHg; p = 0,046), also returning to baseline values after lens removal. In conclusion, this new approach allowed IOP measurement without lens removal. A transient increase in IOP was observed, which returned to baseline values after lens removal, with no significant corneal changes. The technique shows potential for clinical monitoring of SL wearers, particularly in glaucoma patients.eng
